Home Care Packages Program
The Home Care Packages are individually planned and coordinated community care services provided to older people with complex care needs over the age of 65, or those with disability, to live independently in their homes.
FCCVI uses a Consumer Directed Care (CDC) approach where clients have greater choice, flexibility and control over the types of care, and the delivery of those services.
What are Home Care Packages
- Home Care Level 1 provides basic care needs
- Home Care Level 2 provides low level care needs
- Home Care Level 3 provides intermediate care needs
- Home Care Level 4 provides high care needs
Personalized Support for Your Needs
The types of services you can access under a home care package are based on your individual needs, and these are agreed between you and the provider.

To access our CHSP and HCP services, you may also seek help from friendly FCCVI staff with your My Aged Care registration so you can be assessed for supports and services we offer within our organisation.

Schedule of Fees 2023 - 2024 (GST Exclusive)
Overnight Care
- Weekdays - $72/hour
- Saturday - $102/hour
- Sunday - $125/hour
- Public Holiday - $165/hour
Personal Care
Domestic Assistance & Shopping Assistance and Respite Care- Weekdays - $63/hour
- Saturday - $93/hour
- Sunday - $114/hour
- Public Holiday - $144/hour
Nursing Care
Service Delivered By External Provider- Weekdays - $125 /hour
- Saturday - Refer to external provider
- Sunday - Refer to external provider
- Public Holiday - Refer to external provider
- Travel Allowance Per Kilometers - $1.30
- Care Management - 20% of Subsidy
- Package Management - 15% of Subsidy
