Commonwealth Home Support Program
The Commonwealth Home Support Program is an entry level home help program for older people who need some help with daily tasks to live independently at home and in the community. FCCVi offers social support through its Social Support Group (SSG) and Social Support Individual (SSI) services.
Social Support Group (SSG)
Our Damayan Social Support Group is designed to assist elderly clients with participation to community life and social activities. We encourage a haven where anyone can laugh, sing, dance, and thrive.
At present, we have three (3) organised Social Support Groups in Brooklyn, Cragieburn and Morwell. We are looking forward to form more social support groups throughout Victoria.
Social Support Individual (SSI)
Through Damayan Social Support Individual, we provide services to our clients through accompanied activities (various appointments and shopping), home visits, and welfare checks. This service is usually provided on a one-on-one basis but may also be provided to couples.
We endeavour to support you in accessing the community and accompanying you to your activities safely and on time.
Our Services
Social Support Group
One-on-one or group support to help you stay socially engaged. This may include companionship, attending community events, recreational activities, or support to maintain social connections.
Social Support Individual
Assistance with getting to medical appointments, shopping, community activities, and other essential travel to help you stay active and connected.
HACC-PYP Support
HACC-PYP supports eligible younger people with disability to live independently and stay connected to their community through practical, flexible supports that promote choice and independence.

We’ll listen to your situation, answer your questions, and help you understand your eligibility and care needs. We can also assist you with registering for My Aged Care and guide you through the next steps so you know what to expect.
My Aged Care
Assessment
To receive funded services, an assessment is required. We’ll guide you through the referral process, help gather the necessary information, and follow up with you afterward to go through your care plan, funding level and next steps.
Support at Home
Services
When your funding becomes available, simply give us a call and we’ll help you get started. We’ll connect you with a care partner who will visit you at home to create a personalised care plan based on your routines, preferences, and goals. From there, your support journey begins.

Navigating Aged Care
We provide guidance and assistance in understanding and accessing aged care services, including referrals to My Aged Care, care options, assessments and ongoing support through the process.
FCCVI Home Care Services – Support at Home
The Filipino Community Council of Victoria Inc. (FCCVI) is a charitable organisation and home care provider delivering Support at Home services to help older people and people living with disability live safely and independently in their own homes. We offer personalised, culturally respectful care using a Consumer Directed Care approach, giving clients choice and control over their services. FCCVI supports clients with personal care, domestic assistance, transport, social support, allied health, nursing care, and help navigating aged care services.
