Support at Home Program
The Support at Home Program offers individually planned and coordinated care services for people aged 65 and over, and for people living with disability, who need support to remain safe and independent at home. This program replaces what was previously known as the Home Care Package Program.
At FCCVI, we deliver services using a Consumer Directed Care (CDC) approach. This means our clients have greater choice, flexibility and control over the types of care they receive, how the services are delivered, and who provides them. We work closely with each individual to design a care plan that reflects their cultural background, preferences, goals and evolving needs.
Our aim is to support independence, enhance wellbeing, and ensure that every client receives respectful, high-quality care that aligns with their values and lifestyle.
Our Support at Home Services
Personal Care
Support with showering, bathing, grooming, dressing, continence care and personal hygiene—delivered with dignity, privacy and respect.
Transportation and Mobility
Assistance with getting to medical appointments, shopping, community activities, and other essential travel to help you stay active and connected.
Social Support
One-on-one or group support to help you stay socially engaged. This may include companionship, attending community events, recreational activities, or support to maintain social connections.
Nursing and Clinical Care
Professional nursing support for wound care, medication management, chronic disease monitoring and specialised clinical needs to help you stay healthy and safe at home.
Home Assistance
Help with everyday household tasks such as general cleaning, laundry, meal preparation and light gardening to maintain a safe and comfortable home environment.
Allied Health and Therapeutic Services
Access to health professionals such as physiotherapists, occupational therapists, podiatrists and other specialists to support mobility, recovery and overall wellbeing.

My Aged Care
Assessment
To receive funded services, an assessment is required. We’ll guide you through the referral process, help gather the necessary information, and follow up with you afterward to go through your care plan, funding level and next steps.
Support at Home
Services
When your funding becomes available, simply give us a call and we’ll help you get started. We’ll connect you with a care partner who will visit you at home to create a personalised care plan based on your routines, preferences, and goals. From there, your support journey begins.

What is Support At Home?
From 1 November 2025, the Australian Government will introduce a new in-home care system called the Support at Home Program.
This program will replace the Home Care Package (HCP) and Short-Term Restorative Care (STRC) programs, bringing them together into one simpler and more streamlined approach to support older people at home.
What this means for you:
- If you are already receiving care, your services will continue.
- Your current funding and supports will remain in place during the transition.
- The new system will include eight support classifications, along with Restorative and End-of-Life pathways, plus dedicated Assistive Technology and Home Modifications support.
The table below outlines the funding amounts for each of the 8 ongoing service classifications.
These funding amounts include funding for care management.

What is Support at Home Program?
Support at Home is an Australian Government aged care program that helps older people receive coordinated care and support services in their own homes so they can live safely and independently.
Who is eligible for Support at Home?
You may be eligible if you are:
- Aged 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people) and Having trouble managing daily activities at home.
Eligibility is confirmed through an aged care assessment.
How is Support at Home different from Home Care Packages?
Support at Home replaces the Home Care Package program with a simpler system, offering:

Do I need an assessment?
Yes. An assessment is required to determine:
- Your eligibility
- Your funding level
We will follow up with you after your assessment and help you understand your support plan.
Can I choose my services and providers?
Yes. FCCVI follows a Consumer Directed Care (CDC) approach, giving you:
- Choice and flexibility
- Control over your services
- A say in how and when your care is delivered
Can I change providers?
Yes. If you are switching from another provider, FCCVI will:
- Support a smooth transition
- Coordinate with your current provider
- Ensure there is no disruption to your services
Will I have to pay fees?
- Some services may require a client contribution, depending on your income and care needs.
- We will explain any fees clearly before services begin.
What if my needs change?
Your care plan can be reviewed and adjusted as your needs change. FCCVI will work with you to ensure your services continue to meet your goals and preferences.
